The aim of the study was to compare the action of glimepiride and repaglinide in glucose control and basic metabolic parameters of type 2 diabetic patients. Forty four patients (27 females, 17 males), with recently diagnosed type 2 diabetes, were randomized in three groups: DIET group: diet only, GLI: diet plus glimepiride, REP: diet plus repaglinide. We evaluated fasting plasma glucose (FPG), glycosylated haemoglobin (HbA1c), b-cell function and body weight at baseline and after 18 weeks of treatment. In GLI and REP groups there was statistically significant reduction of HbA1c and FPG (p<0.001). Reduction of fasting insulin was observed in GLI and REP groups, but not statistically significant. Significant increase of HOMA-B% was observed in GLI group only (p= 0.035). Body weight and BMI changes were not statistically significant in all groups, but showed more fa- vourable variation in GLI group. In recently diagnosed type 2 diabetes, glimepiride and repaglinide are equally effective in diabetes control but glimepiride increases b-cell function without change in body weight.
