      "@value" : "SUMMARY\nThe peritoneal surface remains an important failure site\nfor patients with colorectal cancer. Peritoneal metastases\nof colorectal cancer are at present considered equal to distant\nmetastatic disease. Consequently, peritoneal carcinomatosis\nis treated with systemic chemotherapy and surgery\nonly to palliate complications such as obstruction. Despite\nthe development of new chemotherapeutic agents and combinations,\nthe results remain disappointing with a limited\nimpact on survival. Colorectal carcinoma cells are relatively\nresistant to chemotherapy. Intraperitoneal chemotherapy\nseems to be an attractive approach in the treatment of highrisk\ncolorectal cancer and peritoneal carcinomatosis of colorectal\norigin providing high local drug concentration with\nlimited systemic side effects. Adjuvant early postoperative\nintraperitoneal chemotherapy is worthwhile considering as\na treatment option after resection of high-risk colorectal\ncancer. Meta-analysis of randomized trials demonstrates\na positive impact of this adjuvant treatment on overall survival\nand regional tumor control. In the treatment of peritoneal\ncarcinomatosis postoperative intraperitoneal chemotherapy\nleads to inadequate exposure of the peritoneal surface.\nOnly intraoperative continuous peritoneal perfusion\nchemotherapy performed with direct cytotoxic drugs such\nas MMC and cisplatin may overcome this problem. The limited  limited\ndrug penetration in tissue implies the need for extensive\ncytoreductive surgery. Additionally, the latter form of\nregional chemotherapy can be performed under hyperthermic\nconditions. Hyperthermia has a direct cytotoxic effect\nand enhances the activity and penetration depth of many\ncytotoxic drugs. The results of phase II studies of cytoreductive\nsurgery and intraoperative hyperthermic intraperitoneal\nchemotherapy for peritoneal carcinomatosis of colorectal\norigin suggest that an increased median survival\ncan be achieved with this approach, especially in patients\nwith no macroscopic or small volume residual disease."
