Οι συγγενείς ή ενδομήτριες λοιμώξεις είναι οι λοιμώξεις που προσβάλλουν το
έμβρυο καθ’ όλη τη διάρκεια της εγκυμοσύνης ή το νεογνό κατά την περιγεννητική
περίοδο.
Οι λοιμώξεις μπορεί να μολύνουν το έμβρυο είτε μέσω της διαπλουντιακής
μεταφοράς, είτε κατά τον τοκετό (από κολπικές εκκρίσεις ή αίμα), ή μετά τη από
τη γέννηση (από το μητρικό γάλα ή άλλες πηγές). Οι κλινικές εκδηλώσεις των
νεογνικών λοιμώξεων ποικίλουν ανάλογα με το παράγοντα που τις προκάλεσε και την
ηλικία κύησης κατά την έκθεση της μητέρας. Ο κίνδυνος μόλυνσης είναι συνήθως
αντιστρόφως ανάλογος με ηλικία κύησης κατά την έκθεση της μητέρας στον
λοιμογόνο παράγοντα
Οι λοιμώξεις της μήτερας και κατ’επέκταση του εμβρύου είναοι λοιμώξεις που
προκαλούν συγγενείς ανωμαλίες και έχουν περιγραφεί με το ακρωνύμιο TORCH
(Toxoplasma, Others, Rubella, Cytomegalovirus, Herpes). Η κατηγορία ‘others’
περιλαμβάνει αρκετούς ιούς που συνδέονται με νεογνικές νόσους.
Η τοξοπλάσμωση προκαλείται από μόλυνση με το πρωτόζωο Toxoplasma
gondii, ένα υποχρεωτικά ενδοκυτταρικό παράσιτο. Περίπου το 10-20% των εγκύων
γυναικών που έχουν προσβληθεί Toxoplasma gondii χαρακτηρίζονται ως
συμπτωματικές. Τα πιο συνηθισμένα συμπτώματα της λοίμωξης είναι η
λεμφαδενοπάθεια και ο πυρετός. Αν η μητέρα έχει μολυνθεί πριν την εγκυμοσύνη,
δεν υπάρχει σχεδόν κανένας κίνδυνος μόλυνσης του εμβρύου, όσο αυτή παραμένει
ανοσοποιημένη. Όταν μια μητέρα μολυνθεί με Toxoplasma gondii κατά τη διάρκεια
της κύησης, το παράσιτο μπορεί να μεταδοθεί αιματογενώς στον πλακούντα.. Ο
κίνδυνος της μόλυνσης στο έμβρυο αυξάνει με την ηλικία κύησης και είναι ήπια ή
δεν είναι εμφανής κατά τη γέννηση.
Η πρωτογενής, η επανενεργοποίηση, ή η υποτροπιάζουσα λοίμωξη από CMV
μπορεί να συμβεί κατά την εγκυμοσύνη και μπορεί να οδηγήσει σε συγγενή CMV
λοίμωξη. Η κάθετη μετάδοση του CMV μπορεί να συμβεί σε οποιοδήποτε στάδιο της
εγκυμοσύνης, ωστόσο, ο μεγαλύτερος κίνδυνος μόλυνσης είναι κατά το τρίτο
τρίμηνο όπου και έχουν αναφερθεί οι σημαντικότερες και σοβαρές επιπτώσεις.
Η ερυθρά ανήκει στην κατηγορία των τερατογόνων ιών.Το συγγενές σύνδρομο
ερυθράς (CRS) εμφανίζεται σε βρέφη που γεννήθηκαν από μητέρες που απέκτησαν τη
λοίμωξη ερυθράς κατά τη διάρκεια των πρώτων 16 εβδομάδων της εγκυμοσύνης.
Ο παρβοϊός B19 προκαλεί ερύθημα λοιμώδες (πέμπτη ασθένεια). Η μόλυνση
εμφανίζεται συχνότερα το χειμώνα και την άνοιξη. Τριάντα έως 40% των εγκύων
γυναικών είναι ορροαρνητικές για το παρβοϊό B19 και έτσι είναι ευαίσθητες σε
πιθανή μόλυνση από τον ιό.Η μόλυνση σε κάθε τρίμηνο μπορεί να οδηγήσει σε
ενδομήτρια απώλεια του εμβρύου.
Περίπου το 10% των μολύνσεωννεογνών από τον ιό HSV είναι εκ γενετής, ως
συνέπεια της απόκτησης πρωτογενούς μόλυνσης κατά την εγκυμοσύνη και τη μεταφορά
είτε μέσω του πλακούντα ή μέσω του τραχήλου της μήτρας. Μια ενδομήτρια μόλυνση
μπορεί να προκαλέσει περιορισμό της ανάπτυξης, πρόωρο τοκετό, και αποβολή.
Η Listeria monocytogenes είναι ιένας βακτηριδιακός παθογόνος
μικροοργανισμός που εκδηλώνεται ως ενδοκυτταρικό παράσιτο συνήθως στα
νεογέννητα, στους ανοσοκατασταλμένους ασθενείς, στα ηλικιωμένα άτομα, σε
εγκύους και περιστασιακά σε υγιή άτομα. Πηγή μόλυνσης είναι ο ζωικές τροφές και
τα υποπροϊόντα τους. Οι κίνδυνοι στην εγκυμοσύνη είναι η αυτόματη αποβολή στο
Α' τρίμηνο, ενδομήτριος θάνατος του εμβρύου και συγγενής νόσηση του νεογέννητου.
Περίπου το 50% των αποικισμένων εγκύων με στρεπτόκοκκο β , θα
αποικιστεί κατά τον τοκετό ή και νωρίτερα , σε περιπτώση πρόωρης ρήξης των
υμενών και 2% από αυτά τα νεογνά θα εμφανίσει λοίμωξη. Ο GBS μπόρει να
προκαλέσει ουρολοίμωξη, βακτηριουρία, χοριοαμνιονίτιδα, ενδομητρίτιδα ή ακόμα
και σηψαιμία .
Η ιογενης ηπατιτιδα ειναι λοιμωξη που αφορα σε νεκρωτικη
φλεγμονη του ηπατος και οφειλεται σε πεντε ιους, Α, Β, C, D, E, πιθανον
και σε εναν εκτο, G. Σε γενικές γραμμές, τόσο η κλινική εικόνα όσο και η
φυσική εξέλιξη της οξείας ιογενούς ηπατίτιδας κατά τη διάρκεια της εγκυμοσύνης
δε διαφέρουν σημαντικά από ότι αναφέρεται στο γενικό πληθυσμό ,με εξαίρεση την
οξεία ηπατίτιδα Ε, ενώ ανάλογες είναι και οι δυνατότητες θεραπευτικής
παρέμβασης η οποία είναι κατά κανόνα συντηρητική.
Οι γυναικες ηλικιας 15-24 ετων ειναι 2,5 φορες πιο πιθανο να
προσβληθουν απο τον ιο HIV απ’ οτι οι ανδρες της ιδιας ηλικιακης
ομαδας. Γι’ αυτο, απο τα 1.700 νεα κρουσματα ΗΙV λοιμωξης σε παιδια που
προκυπτουν καθε μερα παγκοσμιως, τα 2/3 οφειλονται σε μεταδοση απο τη
μητερα στο νεογνο. Η καθετη μεταδοση του ιου HIV-1 απο τη μητερα στο
νεογνο μπορει να συμβει κατα την ενδομητρια, περιγεννητικη η
μεταγεννητικη περιοδο κα ιεχει συσχετισθει κυριως με το ιικο φορτιο στο
μητρικο αιμα, τα επιπεδατων CD4 T-λεμφοκυτταρων, την προωροτητα, τον
τροπο τοκετου, τη διαρκεια ρηξης των εμβρυικων υμενων και το θηλασμο.
Οι λοιμωξεις απο τον ιο των ανθρωπινων θηλωματων (Human Papilloma
Virus - HPV) αποτελουν τις συχνοτερες σεξουαλικα μεταδιδομενες λοιμωξεις
και την αναγκαια προυποθεση για την αναπτυξη δυσπλασιας η καρκινου του
τραχηλου της μητρας. Σημερα εχουν εγκριθει προληπτικα εμβολια εναντι
της HPV λοιμωξης, τα οποια αναμενεται να μειωσουν σημαντικα την επιπτωση
του καρκινου του τραχηλου της μητρας τις επομενες δεκαετιες.
Οι διάφοροι μικροοργανισμοί οπώςτο M. hominis,το U. Urealyticum,
τοπαράσιτο Trichomonas vaginalis, οι μύκητες (Candida albicans) και η
Gardnerella Vaginalis, προκαλούν κολπίτιδα και τραχηλίτιδα.
Η βλεννόρροια ή γονοκοκκική ουρηθρίτιδα οφείλεται σε Gramm(-)
διπλόκοκκο, το γονόκοκκο (Neisseria gonorrhoeae). Η νόσος παρουσιάζεται κατά
την κύηση σε ποσοστό 0,5-7% και το ποσοστό των ασυμπτωματικών μορφών της είναι
75-90%. Η νόσος προκαλεί επιπλοκές στη μητέρα, το έμβρυο και το νεογνό. Είναι
αναγκή να γίνει έγκαιρη διάγνωση, κατάλληλη αντιμετώπιση και κυρίως , πρόληψη
των λοιμώξεων αυτών στην έγκυο και το νεογνό.
Οι ενδεχόμενες επιπτώσεις στο έμβρυο απο αυτές τις λοιμώξεις μπόρει να
είναι η υπολειπόμενη αναπτυξή, οι συγγενείς ανωμαλίες διάπλασεις, η λειτουργική
ανεπάρκεια διαφόρων οργάνων ή αλλές φόρες ο θάνατος.

Viral infections in pregnancy are major causes of maternal and fetal morbidity
and mortality. Infections can develop in the neonate transplacentally,
perinatally (from vaginal secretions or blood), or postnatally (from breast
milk or other sources). The clinical manifestations of neonatal infections vary
depending on the viral agent and gestational age at exposure. The risk of
infection is usually inversely related to gestational age at acquisition, some
resulting in a congenital malformation syndrome. Infections known to produce
congenital defects have been described with the acronym TORCH (Toxoplasma,
others, rubella, cytomegalovirus [CMV], herpes). The "others" category has
rapidly expanded to include several viruses known to cause neonatal disease.
Traditionally, the viral infections of concern during pregnancy were those
caused by rubella virus, CMV, herpes simplex virus (HSV), and parvovirus B19
(B19V).
Toxoplasmosis is caused by infection with the protozoan Toxoplasma gondii, an
obligate intracellular parasite. Approximately 10-20% of pregnant women
infected with T gondii become symptomatic. The most common signs of infection
are lymphadenopathy and fever. If the mother was infected prior to pregnancy,
there is virtually no risk of fetal infection, as long as she remains
immunocompetent. When a mother is infected with T gondii during gestation, the
parasite may be disseminated hematogenously to the placenta. When this occurs,
infection may be transmitted to the fetus transplacentally or during vaginal
delivery. The risk of infection to the fetus increases with the gestational age
and is mild or not apparent at birth. The most significant manifestation of
toxoplasmosis in the fetus is encephalomyelitis, which may have severe results.
Approximately 10% of prenatal T gondii infections result in abortion or
neonatal death. In approximately 67-80% of prenatally infected infants, the
infection is subclinical and can be diagnosed using only serological and other
laboratory methods. Although these infants appear healthy at birth, they may
develop clinical symptoms and deficiencies later in life.
Vertical transmission of CMV can occur at any stage of pregnancy; however,
severe sequelae are more common with infection in the first trimester, while
the overall risk of infection is greatest in the third trimester. Most (90%)
CMV infections cause no symptoms, but 10% result in signs and symptoms, such as
congenital hearing loss (most common). However; thirty percent of infants with
severe CMV infection die; among survivors, more than half eventually develop
neurological sequelae, including microcephaly, mental retardation, and/or
sensorineural hearing loss.
Rubella is one of the more teratogenic viruses. Congenital rubella syndrome
(CRS) is characterized by intrauterine growth restriction, intracranial
calcifications, microcephaly, cataracts, cardiac defects, neurologic disease,
osteitis, and hepatosplenomegaly. Most of these complications develop in
infants born to mothers who acquire rubella infection during the first 16 weeks
of pregnancy.
B19V causes erythema infectiosum (fifth disease). Although most adults with
B19V infection are asymptomatic, the effects of this virus on the fetus are
much greater and include miscarriage, fetal anemia, hydrops fetalis,
myocarditis, and/or intrauterine fetal death. Infection occurs most frequently
in the winter and spring. Thirty to 40% of pregnant women are seronegative for
B19V and are thus susceptible to infection. Second-trimester infections have
been studied most frequently because infection in this trimester carries a
1%-3% risk of hydrops; however, infection in any trimester may result in
intrauterine fetal loss.
Thirty to 60% of women receiving obstetric care have serologic evidence of past
HSV infection. Approximately 10% of infections are congenital, usually a
consequence of the mother acquiring primary HSV infection during pregnancy and
the fetus acquiring the infection transplacentally or via an ascending
infection from the cervix. Intrauterine infection is associated with
intrauterine growth restriction, preterm labor, and miscarriage. The risk of
neonatal herpes and death is highest in infants born to mothers who have not
seroconverted by the time of delivery.
Approximately 50 % of pregnant women colonized with strep b be colonized at
birth or earlier, in the case of premature rupture of membranes and 2 % of
these infants will show infection . The GBS can cause urinary tract infection,
bacteriuria , chorioamnionitis , endometritis or even septicemia .
Viral hepatitis infection is directed to necrotizing inflammation of the liver
and is due to five viruses, A, B , C, D, E, and possibly a sixth , G. In
general , both the clinical picture and the natural evolution of acute viral
hepatitis during pregnancy does not significantly differ from that reported in
the general population , with the exception of acute hepatitis C , while
analogous is the potential for therapeutic intervention which is generally
conservative .
Women aged 15-24 are 2.5 times frequently to be infected with HIV than men of
the same age group . Therefore, the 1,700 new cases of HIV infection in
children occur every day worldwide, the 2/3 due to transmission from mother to
infant. Vertical transmission of HIV-1 virus from mother to infant may occur in
utero , perinatal or postnatal period and has been associated mainly with viral
load in maternal blood levels of CD4 T- lymphocytes , prematurity , mode of
birth , duration of rupture of fetal membranes and breastfeeding .
Infections by the human papilloma virus (Human Papilloma Virus - HPV) are the
most common sexually transmitted infections and the necessary condition for the
development of dysplasia or cancer of the cervix . Currently approved
prophylactic vaccines against HPV infection , which is expected to
significantly reduce the incidence of cancer of the cervix in the coming
decades .
Different microorganisms such as M. hominis, the U. Urealyticum, the parasite
Trichomonas vaginalis, fungi (Candida albicans) and Gardnerella Vaginalis,
causing vaginitis and cervicitis .
The gonorrhea or gonococcal urethritis due to Gramm (-) diplococci , the
gonococcus (Neisseria gonorrhoeae). The disease occurs in pregnancy rate at
0.5-7 % and the proportion of asymptomatic forms is 75-90 %. The disease causes
complications to the mother, the fetus and neonate . Need to make early
diagnosis , appropriate treatment and especially prevention of these infections
in pregnant women and newborn .
The potential impact on the fetus from these infections may be the remaining
development , congenital anomalies of conformation , the functional impairment
of various organs or sometimes death .
