Σκοπός : Να ερευνήσουμε εάν το νοσηλευτικό προσωπικό μπορεί να διασωληνώσει και
με ποιο ποσοστό επιτυχίας, χρησιμοποιώντας την ilma και το igel. Παράλληλα θα
μελετήσουμε την επίδραση των θωρακικών συμπιέσεων στην απόδοσή τους και με τις
δυο υπεργλωττιδικές συσκευές.
Υλικό και Μέθοδος : Σαράντα πέντε νοσηλευτές , χωρίς προηγούμενη εμπειρία στη
χρήση της ilma και του igel συμμετείχςαν στη μελέτη. Μετά από μια σύντομη
επίδειξη των δύο συσκευών, οι νοσηλευτές κλήθηκαν να πραγματοποιήσουν τυφλή
ενδοτραχειακή διασωλήνωση σε πρόπλασμα κατά η διάρκεια θωρακικών συμπιέσεων και
χωρίς θωρακικές συμπιέσεις χρησιμοποιώντας και τις δυο συσκευές. Μετρήθηκαν η
χρονική διάρκεια και τα ποσοστά επιτυχίας της διασωλήνωσης.
Αποτελέσματα: Η μέση χρονική διάρκεια διασωλήνωσης χωρίς διενέργεια θωρακικών
συμπιέσεων ήταν 20.60 ± 3.27s για την ilma και 18.40 ± 3.26s ( p<0.0005) για
το igel. Όταν οι θωρακικές συμπιέσεις δεν διακόπτονταν η χρονική διάρκεια ήταν
μικρότερη για το igel σε σύγκριση με την ilma (20.00 ± 3.71s vs 21.99 ± 3.62s
, p<0.0005).Οι συνεχιζόμενες θωρακικές συμπιέσεις παρέτειναν το χρόνο
διασωλήνωσης και με τις δυο συσκευές( 20.00 ± 3.71s vs 18.40s χωρίς ΘΣ για την
igel, p<0.0005 και 21.99 ± 3.62s vs 20.60 ± 3.27s χωρίς ΘΣ για την ilma,
p<0.0005 ) χωρίς να επηρεάζουν σημαντικά τα ποσοστά επιτυχίας(77.8% vs
86.7% χωρίς ΘΣ για τηνilma, p=0.272 και73.3% vs 82.2% χωρίς ΘΣ για την igel,
p=0.313 ).
Συμπέρασμα:Συμπερασματικά, η μελέτη αυτή έδειξε ότι τόσο η ILMA όσο και η i-gel
μπορούν να χρησιμοποιηθούν με την ίδια επιτυχία για την τυφλή διασωλήνωση της
τραχείας από νοσηλευτές χωρίς προηγούμενη εμπειρία. Παράλληλα, η ενδοτραχειακή
διασωλήνωση μπορεί να επιτευχθεί χωρίς τη διακοπή των θωρακικών συμπιέσεων. Ο
αεραγωγός i-gel φάνηκε ότι είναι εξίσου αποτελεσματικός με την ILMA και η χρήση
του σχετίζεται με ταχύτερους χρόνους διασωλήνωσης. Παρόλα αυτά, προτείνουμε την
περαιτέρω αξιολόγηση των επιδόσεων της ILMA και του i-gel κατά τη διάρκεια της
καρδιοαναπνευστικής αναζωογόνησης σε κλινικό επίπεδο.

Objective: To investigate whether nursing staff can successfully ventilate and
intubate using the i-gel and the intubating laryngeal mask ILMA Fastrach,
during cardiac arrest.
Background: Airway management during cardiopulmonary resuscitation is a crucial
step. Tracheal intubation is considered to be the optimal method for securing
the airway, but interventions such as laryngoscopy require a high level of
skill and should be undertaken by experienced personnel only. However,
placement of supraglottic devices is easier and can be carried out by
inexperienced rescuers in the setting of cardiac arrest. The intubating
laryngeal mask (ILMA) Fastrach provides an alternative way to tracheal
intubation. Recent studies suggest that the i-gel apart from its utility in
ventilation can be used as a conduit for intubation as well.
Methods: Forty-five nurses, not experienced in the use of the ILMA and the
i-gel were enrolled in our study. After a brief demonstration of both devices,
they were asked to perform endotracheal intubation using the i-gel and the ILMA
in a manikin, with continuous and without chest compressions. A size 4.0 ILMA,
a size 4.0 i-gel and the appropriate for each device endotracheal tubes size
7.0mm (ID, internal diameter) were used. Duration of intubation and success
rates for each device, with and without chest compressions were our primary
endpoints.
Results: Mean intubation times for the ILMA and i-gel without chest
compressions were 20.60 ± 3.27s and 18.40 ± 3.26s ( p<0.0005) respectively.
When intubation was attempted without interrupting chest compressions, mean
times for i-gel were faster than for the ILMA (20.00 ± 3.71s vs 21.99 ± 3.62s ,
p<0.0005). Overall success rates did not differ significantly between the two
devices, but the ILMA proved more successful than the i-gel regardless of
compressions. The ILMA had 91.9% and the i-gel 83.7% success rates without
compressions (p=0.062) while with continuous compressions success rates were
84.4% and 82.2% respectively (p=0.744). Continuation of compressions caused a
prolongation in intubation times for both the i-gel ( 20.00 ± 3.71s vs 18.40s
without compressions, p<0.0005) and the ILMA ( 21.99 ± 3.62s vs 20.60 ± 3.27s
without compressions, p<0.0005). However, success rates were not significantly
affected by chest compressions either for the ILMA (77.8% vs
86.7% without compressions, p=0.272) or for the i-gel (73.3% vs 82.2% without
compressions, p=0.313).
Conclusion: Nursing staff can successfully intubate using the i-gel and the
ILMA as conduits with comparable success rates, regardless of whether chest
compressions are interrupted or not. Moreover, blind intubation through the
i-gel is faster than the ILMA in both scenarios (with and without
compressions). However, results from this manikin study should be verified in
the clinical setting.
