Lung mechanics at rest and during exercise in obese people
Μηχανική των πνευμόνων σε ηρεμία και άσκηση παχύσαρκων ασθενών

Σκοπός της μελέτης: Να εμβαθύνουμε τη γνώση μας στη μηχανική των πνευμόνων σε ηρεμία και άσκηση παχύσαρκων ατόμων και να μελετήσουμε την παρουσία ή μη του περιορισμού της εκπνευστικής ροής κατά τη διάρκεια της ηρεμίας και της άσκησης. Υλικό και μέθοδος: Μελετήθηκαν 8 άτομα ηλικίας (mean±SD) 37±11 ετών με δείκτη μάζας σώματος (BMI) 25±3 kg/m2 (ομάδα ελέγχου-0), 10 άτομα ηλικίας 46±11 ετών με BMI 35±3 (ομάδα 1) και 10 άτομα ηλικίας 40±5 ετών με BMI 48±4 (ομάδα 2). Την πρώτη ημέρα της μελέτης υποβλήθηκαν σε σπιρομέτρηση, στατικούς όγκους και διαχυτική ικανότητα των πνευμόνων. Αέρια αρτηριακού αίματος ελήφθησαν στις ομάδες 1 και 2 και έγινε έλεγχος της λειτουργίας των αναπνευστικών μυών. Η χρόνια δύσπνοια εκτιμήθηκε με την κλίμακα mMRC. Ακολούθησε δοκιμασία μέγιστης άσκησης σταδιακά αυξανόμενου έργου (20 W/min) σε εργομετρικό ποδήλατο με την οποία καθορίσθηκε η μέγιστη ικανότητα για άσκηση (WRmax). Ο περιορισμός της εκπνευστικής ροής (EFL) ανιχνεύθηκε με τη μέθοδο NEP (negative expiratory pressure). Τη δεύτερη ημέρα έγινε σπιρομέτρηση σε καθιστή, ύπτια και καθιστή στάση στο ποδήλατο και εφαρμόσθηκε η τεχνική NEP στις ίδιες θέσεις και στο 33%, 66% και 90% του μέγιστου έργου. Αποτελέσματα: Τα άτομα με σοβαρή παχυσαρκία (BMI >40 kg/m2 ) είχαν μειωμένη FRC σημαντικά χαμηλότερο ERV και περισσότερη δύσπνοια σε χρόνια βάση σε σχέση με τις ομάδες Ι και 2 (p<0,05). H ικανότητα για άσκηση (WRmax) ήταν χαμηλότερη στην ομάδα 2 αλλά εντός φυσιολογικών ορίων κυμάνθηκαν όλοι οι παχύσαρκοι. Η μέγιστη κατανάλωση οξυγόνου σε απόλυτη τιμή ήταν αυξημένη στην ομάδα 2. Σε ύπτια θέση παρουσίασαν EFL 6 άτομα στην ομάδα 2 έναντι των 3 στην ομάδα 1. Κατά την άσκηση EFL παρουσίασε μόνο η ομάδα 2 (6 άτομα). Η εισπνευστική χωρητικότητα (IC) παρουσίασε σημαντική θετική συσχέτιση με το WRmax. Η mMRC συσχετίσθηκε σημαντικά με το βάρος και τον ΒΜΙ και το WRmax με τους ίδιους δείκτες αλλά αρνητικά. Συμπεράσματα: Τα μη ηλικιωμένα άτομα με σοβαρή παχυσαρκία εμφανίζουν χαμηλότερη ικανότητα για άσκηση αλλά εντός φυσιολογικών ορίων. Εμφανίζουν επίσης συχνότερα EFL σε ύπτια θέση σε σχέση με τους παχύσαρκους (BMI<40 kg/m2) και EFL κατά την άσκηση, φαινόμενο το οποίο απουσιάζει από όλους τους άλλους. Με την αύξηση του βάρους αυξάνεται η δύσπνοια σε χρόνια βάση και ελαττώνεται το WRmax. Η IC σχετίζεται σημαντικά με την ικανότητα για απόδοση έργου.
Aim of the study: Our study was conducted to extent our knowledge on ventilatory mechanics at rest and during exercise in obese subjects and to detect the presence or absence of expiratory flow limitation at rest and during exercise. Methods: Eight subjects with mean age (mean±SD) 37±11 years and mean BMI 24±3 kg/m2 (GROUP 0), ten obese with mean age 46±11 years and mean BMI 35±3 (GROUP 1) and ten massively obese with mean age 40±5 years and mean BMI 48±4 (GROUP 2) were studied. On the first visit all subjects underwent forced spirometry, static lung volumes and carbon monoxide transfer factor (DLCO). Maximum inspiratory (PImax) and expiratory (PEmax) mouth pressures were also measured. Arterial blood gases were measured only in the obese subjects (Group 1& 2). Evaluation of chronic dyspnoea was made by using the modified MRC scale. Incremental symptom-limited exercise testing (20 Watt/min) was conducted on an electrically braked cycle ergometer to determine the maximum power output (WRmax). The presence of expiratory flow limitation (EFL) was detected by using the negative expiratory pressure technique (NEP). On the second day spirometry was conducted in seated, supine and sitting on the bicycle positions. NEP was applied at the same positions and while cycling at 33%, 66% and 90% of the WRmax. Results: Severe obese subjects (BMI >40 kg/m2 ) exhibited reduced FRC lower ERV and increased chronic dyspnea (mMRC) compared to the other groups (p<0,05). Exercise capacity (WRmax) was reduced in GROUP 2 but this was still within normal limits for all the participants. The absolute value of V΄O2max was increased in GROUP 2. Six subjects in the GROUP 2 were EFL in the supine position vs 3 in GROUP 1. During exercise EFL was present only in GROUP 2 (6 subjects). Inspiratory capacity (IC) was positively and significantly correlated with WRmax. mMRC was significantly correlated with body weight and BMI and those two negatively with WRmax. Conclusions: Young massively obese people exhibit reduced exercise capacity but are still within normal limits. They exhibit more often EFL in the supine position compared to obese with BMI<40 kg/m2. EFL at exercise is present only in massively obese people. The increased body weight leads to more chronic dyspnoea and reduced WRmax. IC is well correlated with exercise capacity.
