Systematic exrcise training and the function of vessels in patients with diabetes mellitus

Σκοπός της παρούσας διατριβής, ήταν να εξετάσει σε ασθενείς με σακχαρώδηδιαβήτη (ΣΔ) τύπου 2 i) την επίδραση ενός συστηματικού προγράμματος άσκησης σεφλεγμονώδεις και αντιφλεγμονώδεις παράγοντες, ii) την οξεία επίδραση της δοκιμασίαςκόπωσης στη διακύμανση καρδιακής συχνότητας (ΔΚΣ), και την προσαρμογή της έπει-τα από ένα πρόγραμμα άσκησης, iii) την επίδραση ενός προγράμματος άσκησης, δια-κοπής και επανάληψης του σε φυσιολογικές παραμέτρους ασθενών με ΣΔ τύπου 2 καιiv) το βαθμό ανάπτυξης της ασβέστωσης στην αορτή σε επιμύες με ΣΔ τύπου 2 ανάλο-γα με την περιεκτικότητα σε λίπος στη διατροφή και τη φυσική δραστηριότητα. Παρα-τηρήθηκε βελτίωση στα ανθρωπομετρικά χαρακτηριστικά, στο γλυκαιμικό έλεγχο καισε παραμέτρους φυσικής κατάστασης καθώς και αντιφλεγμονώδη δράση. Κατά την ο-ξεία άσκηση οι μεταβολές υψηλής συχνότητας (HF) των διαστημάτων R-R αποκατά-στασης μειώθηκαν. Το συστηματικό πρόγραμμα άσκησης αύξησε σημαντικά το χρόνοδοκιμασίας κόπωσης, ενώ υπήρξε μια τάση αύξησης των HF αποκατάστασης. Η διακο-πή ενός συστηματικού προγράμματος άσκησης ανέδειξε αντιστροφή των θετικών προ-σαρμογών, ενώ η επανάληψη του ίδιου προγράμματος ανέκτησε και βελτίωσε ακόμαπερισσότερο τα θετικά αποτελέσματα. Επίμυες (n=45) με διατροφή 10% ή 60% σε λί-πος ή /και φυσική δραστηριότητα, παρουσιάσθηκε μείωση της έκτασης αθηροσκλήρω-σης των επίμυων που ασκούνταν είτε ακολουθούσαν χαμηλή (10%), είτε υψηλή (60%)σε λίπος διατροφή. Τα παραπάνω έδειξαν ότι η συστηματική άσκηση επιφέρει ρύθμισητης υπεργλυκαιμίας και καταστολή της χρόνιας υποκλινικής φλεγμονής, βελτιώνει τηλειτουργία των αγγείων, επιταχύνει την αποκατάσταση της καρδιακής συχνότητας καιομαλοποιεί την ΔΚΣ ασθενών με ΣΔ τύπου 2. Το ποσοστό ασβέστωσης της βαλβίδαςτης αορτής παρουσιάζει ενδείξεις βελτίωσης μετά από άσκηση. Σημαντικό είναι τοπρόγραμμα άσκησης να διεξάγεται σε μόνιμη και συστηματική βάση προκειμένου οιασθενείς να βελτιώνουν και να σταθεροποιούν την κλινική τους εικόνα.
The purpose of this dissertation was to examine the effect of i) systematic exercisetraining on inflammatory and anti-inflammatory parameters, ii) acute stress test andtraining on heart rate variability (HRV), iii) exercise training, detraining and retrainingon physiological parameters in patients with type 2 diabetes and iv) voluntary exerciseon calcification of aortic valve in mice with type 2 diabetes. Patients with type 2 DM(n=10) participated in a supervised exercise training program and an improvement ofsomatometrics, glycemic control (HbA1c:-11.8%, p<0.05) and physical fitness parameterswere observed as well as an anti-inflammatory effect. The acute response of heartrate variability (HRV) was measured after a stress test in patients with type 2 diabetes(n=12) and a decrease of high frequency power (HF) was observed. Patients (n=6) whoenrolled on a systematic exercise training program of 2 months, decreased the durationof stress test and increased recovery HF. A systematic exercise training program of ninemonths, which combines aerobic and resistance exercise, had positive effects on somatometrics,biochemical parameters and physical fitness of patients with type 2 diabetes(n=13). Three months of detraining reversed the physiological adaptations, whereas theresumption of training improved further the initial training adaptations. C57BL/6 mice(n=45) were randomly divided in 4 groups of low or high fat diet and physically activeor inactive. There was a reduction in the calcification of aortic valve of both physicalactive groups, with either low (10%), or high (60%) fat diet. The above results indicatethat systematic exercise training which combines aerobic exercise and strength trainingcontributes to glucose control and decrease of chronic inflammation, improves heartrate recovery and HRV of patients with type 2 diabetes. Aortic valve calcification decreasesin mice that exercise whether they have low or high fat diet. Diabetic patientshave to participate in regular exercise in order to control glucose metabolism and improvetheir health status.
