Σκοπός της παρούσας μελέτης είναι η διερεύνηση και η σύγκριση της γωνίας τού πρόσθιου θαλάμου, με τη βοήθεια της αξονικής τομογραφίας του πρόσθιου θαλάμου (Visante™ OCT), πριν και μετά από επέμβαση καταρράκτη. Εκτιμάται ότι οι πληροφορίες, που θα προκύψουν μετά από αυτή τη σύγκριση, θα μας οδηγήσουν στην καταγραφή του πραγματικού εύ¬ρους της γωνίας (εικ. 12). Η κύρια υπόθεση του ερευνητικού πρωτοκόλλου είναι η ακόλουθη: μετά την αφαίρεση του φακού και την τοποθέτηση ενδοφακού οπίσθιου θαλάμου, η γωνία του πρόσθιου θαλάμου θα βρεθεί μεγαλύτερη. Παράλληλα, θα γίνει καταγραφή και μελέτη όλων των ανατομικών και μορφολογικών αλλαγών που θα προκύψουν μετά την αφαίρεση του κρυσταλλοειδούς φακού και την ένθεση ενδοφακού. Επίσης, αυτή η έρευνα θα συμβάλλει στην καλύτερη κατανόηση της διακύμανσης της γωνίας και των παθοφυσιολογικών μηχα¬νισμών της, ώστε να βοηθήσει στη διαμόρφωση μιας καλύτερης μελλοντικής θεραπευτικής στρατηγικής, ιδίως σε γλαυκωματι¬κούς ασθενείς με πολύ μικρή γωνία πρόσθιου θαλάμου. Changes in angle morphology post cataract surgery can be imaged and objectively quantified by anterior segment OCT. Clinical experience has demonstrated that cataract extraction causes deepening of the central anterior chamber (AC) and widening of the angle. It is also common clinical understanding that as lens thickness (LT) increases, there is an increase in angle crowding with predisposition to relative pupillary block. Thus cataract extraction has been advocated in eyes with primary angle closure. Gonioscopy has traditionally been used for AC angle grading, but is subjective and limited. Ultrasound biomicroscopy (UBM) is a more objective and reproducible method of angle assessment, but the immersion requirement is inconvenient and may cause artificial angle widening. Optical coherence tomography (OCT) is an imaging modality that allows cross-sectional imaging of the eye. It requires no contact or immersion and produces images with higher spatial resolution. We used OCT to quantify changes in anterior segment morphology after cataract extraction and to evaluate the effect of lens thickness and position on angle opening. OCT images demonstrate that after cataract extraction, the convex iris configuration flattens and AC depth and angle widen. These changes can be quantified using custom soft¬ware that has been previously validated in comparison to UBM and gonioscopy. Cataract surgery deepen the anterior chamber significantly, it widens the iridocorneal angle and lowers intraocular pressure. The results were more significant in group of patients with closed-angle glaucoma.
