Το πρώτο ατύχημα είναι το Selby Rail Disaster και συνέβη στο Great Heck στις 28 Φεβρουαρίου του 2001, το οποίο περιλαμβάνει τον εκτροχιασμό και την σύγκρουση δύο αμαξοστοιχιών που προκλήθηκε από την είσοδο οχήματος στον σιδηροδρομικό χώρο και τη σύγκρουση του πρώτου τρένου με αυτό. Το δεύτερο ατύχημα αφορά αυτό του Grayrigg που συνέβη στις 23 Φεβρουαρίου του 2007, ο εκτροχιασμός του οποίου προκλήθηκε από βλάβη σε κινητό σημείο της σιδηροδρομικής γραμμής. Τα ατυχήματα οφείλονται σε ανθρώπινο λάθος, κάτι το οποίο προκύπτει έπειτα από την χρήση των δύο μεθόδων για την συστημική ανάλυση και χαρτογράφηση των συμβάντων. Ο σκοπός της διπλωματικής είναι να εξετάσει τα προβλήματα που υπάρχουν στην εφαρμογή αυτών των τεχνικών διερεύνησης καθώς και να προτείνει ένα τρόπο ενσωμάτωσης αυτών των δύο τεχνικών στο ίδιο πλαίσιο ανάλυσης.The subject of this thesis deals with the analysis of two rail accidents in the UK using the STAMP and ACCIMAP methodologies. The first accident is the Selby Rail Disaster and occurred at Great Heck on 28 February 2001, which involves the derailment and collision of two trains caused by a vehicle entering the rail yard and the first train colliding with it. The second accident concerns the Grayrigg accident which occurred on 23 February 2007, the derailment of which was caused by a fault in a moving part of the railway line. The accidents were caused by human error, which is the result of using the two methods for the systemic analysis and mapping of incidents.
