      "@value" : "Microcephaly is defined as an occipitofrontal head circumference (OFC) which is \n2 or more standard deviations below the mean (-2SD) for age and sex. It results \nfrom the premature fusion of the cranial sutures due to impaired neurogenesis. \nMicrocephaly can be either isolated or it may coexists with other neurological \nentities and/or multiple congenital anomalies, known as syndromic \nmicrocephaly.  Although many syndromic cases can be classified based on the \ncharacteristic phenotype, some others require further investigation. The \npresent study describes the clinical and laboratory investigation of children \nwith syndromic microcephaly and the application of array-comparative genomic \nhybridization (array-CGH) on the unknown cases in order to identify clinically \nrelevant copy number variations (CNVs).  \nFrom a cohort of 203 unrelated patients with syndromic microcephaly who were \nreviewed in the Clinical Genetics Outpatient Clinic, 60 undiagnosed cases \nunderwent array-CGH analysis with the informed consent of the parents. In all \ncases previous standard karyotype as well as other genetic tests were negative. \nHigh resolution 4x180K and 1x244K Agilent arrays (>170.000 and >236.000 probes \nrespectively, average resolution 7-13Kb) were used in this \nstudy.                                                                          \n                                   \nIn 38 out of 60 patients featuring microcephaly among other phenotypic \nanomalies, array-CGH revealed significant CNVs ranging in size from 0.008 to \n31.6Mb. In 21 of the 38 patients, complex rearrangements contributed to the \noverall clinical phenotype and a total of 41 microdeletions and 18 \nmicroduplications were identified. Microcephaly was accompanied by \ndevelopmental delay/intellectual disability in almost all patients (37/38 \npatients). Other associated symptoms included: dysmorphic features in 33, \nepilepsy in 14, hypotonia in 13, congenital heart disease in 7 and other \ncongenital anomalies in 12. Magnetic resonance imaging (MRI) was pathogenic in \n22 out of the 38 patients indicating a variety of findings. It is worthwhile to \nmention that 16 array-CGH positive patients presented with congenital \nmicrocephaly and 15 presented with postnatal microcephaly. In the remaining 7 \nout of the 34 patients, the time of onset remains unknown. Finally, significant \ngenes which may contribute to the underdevelopment of the brain were identified \nin 27 patients\nIn conclusion, array-CGH contributes to the elucidation of undefined syndromic \nmicrocephalic cases, by permitting the discovery of plausible novel \nmicrodeletion and/or microduplication syndromes and contributing to more \nprecise phenotype- genotype correlation."

      "@value" : "Εισαγωγή: Ως μικροκεφαλία ορίζεται η κατάσταση η οποία χαρακτηρίζεται από \nπερίμετρο κεφαλής μικρότερη των δύο σταθερών αποκλίσεων από τη μέση τιμή  ή \nμικρότερη από την 3η εκατοστιαία θέση ανάλογα με την ηλικία  και το φύλο του \nασθενούς. \nΣκοπός: Η μικροκεφαλία μπορεί να είναι είτε μεμονωμένη, είτε συνδρομική. Η \nπαρούσα διδακτορική διατριβή περιγράφει την κλινική και εργαστηριακή διερεύνηση \nτων ασθενών με μικροκεφαλία που προσήλθαν στα εξωτερικά ιατρεία της Κλινικής \nΓενετικής του Εργαστηρίου Ιατρικής Γενετικής του Πανεπιστημίου Αθηνών από \n12/2009 έως 06/2014 και την εφαρμογή της μεθόδου γενωμικού συγκριτικού \nυβριδισμού (array-CGH) στις αδιάγνωστες περιπτώσεις. \nΥλικό – Μέθοδος: Από τους 203 ασθενείς με συνδρομική μικροκεφαλία που προσήλθαν \nστα εξωτερικά ιατρεία της Κλινικής Γενετικής, η μέθοδος  array-CGH εφαρμόστηκε \nσε 60 αδιάγνωστες περιπτώσεις, μετά από γραπτή συγκατάθεση των γονέων. \nΧρησιμοποιήθηκαν υψηλής ανάλυσης 4x180K και 1x244K Agilent arrays με μέση \nανάλυση 7-13Kb.            \nΑποτελέσματα: Σε 38 ασθενείς αναγνωρίστηκαν παθολογικά CNVs μεγέθους \n0.008Mb-34Μb.   Σε 21 από τους 38 ασθενείς, περισσότερες από μία χρωμοσωμικές \nαναδιατάξεις συνέβαλλαν στη κλινική έκφραση του φαινοτύπου. \nΑπό τα συνολικά αποτελέσματα: Η μικροκεφαλία συνοδεύονταν από ψυχοκινητική \nή/και διανοητική καθυστέρηση (n=37), δυσμορφικά χαρακτηριστικά (n=33), επιληψία \n(n=14), κεντρική υποτονία (n=13), συγγενής καρδιοπάθεια και άλλες συγγενείς \nανωμαλίες (n=7, n=12 αντίστοιχα). Η μαγνητική τομογραφία εγκεφάλου (MRI) ήταν \nπαθολογική σε 22 ασθενείς. Σημειώνεται συγγενής μικροκεφαλία (n=16), ενώ \nεπίκτητη (n=15). Τέλος σημαντικά γονίδια που πιθανώς συμβάλλουν στην εμφάνιση \nτης μικροκεφαλίας αναγνωρίστηκαν σε 27 ασθενείς.\nΣυμπεράσματα: Η εφαρμογή της μεθόδου array-CGH σε αδιάγνωστες περιπτώσεις \nσυνδρομικής μικροκεφαλίας είναι χρήσιμη και σημαντική, διότι επιτρέπει την \nαναγνώριση μικροελλειμμάτων και μικροδιπλασιασμών που πιθανότατα ευθύνονται για \nτον φαινότυπο του ασθενούς και επίσης συμβάλλει στη καλύτερη συσχέτιση \nγονοτύπου-φαινοτύπου."
