Due to its low \nfrequency, the biological and clinical features of MBL remain unexplored. \nThe present study included 171 high count MBL individuals. The main goal of the \nstudy was the understanding of the biology of MBL and the mechanisms underlying \nthe progression from MBL to overt leukemia. \nOur findings highlight that there is a high heterogeneity among MBL cases \nregarding their haematological, immunophenotypic and cytogenetic features. On \nthe other hand, the comparison of MBL characteristics with features of CLL \npatients indicate that there may be a common biology and common mechanisms \nunderlying the progression from MBL to overt leukemia.\nFollow-up monitoring revealed two different patterns of B-cell kinetics and the \npresence of clonal evolution at the preclinical state. MBL cases with clonal \nevolution carried a higher risk of progression to overt disease. Moreover, the \nincreased numbers of absolute B cell count at diagnosis could predict a shorter \ntime to progression to frank leukemia. In multivariable analysis both absolute \nB cell count at diagnosis and the presence of clonal evolution, retained their \nindependent prognostic value." }, { "@language" : "el", "@value" : "Η Μονοκλωνική Β Λεμφοκυττάρωση (ΜΒL) αποτελεί την πρόδρομο μορφή της Β Χρόνιας \nΛεμφικής Λευχαιμίας (XΛΛ) και πιθανότατα και άλλων χρόνιων λεμφοϋπερπλασιών του \nπεριφερικού αίματος. Λόγω της μικρής συχνότητας εμφάνισης της διαταραχής η \nκλινική σημασία της MBL και τα βιολογικά της χαρακτηριστικά παραμένουν \nανεπαρκώς τεκμηριωμένα.\nΣτην παρούσα εργασία αναλύθηκαν 171 MBL περιστατικά «υψηλού αριθμού». Ο βασικός \nσκοπός της εργασίας ήταν η κατανόηση της βιολογίας της MBL και η κατανόηση των \nμηχανισμών που εμπλέκονται στη μετάβαση από το προκλινικό, στο κλινικό στάδια \nτης νόσου. \nΤα αποτελέσματα μας αναδεικνύουν ότι υπάρχει μία πολύ μεγάλη ετερογένεια μεταξύ \nτων MBL περιστατικών σε αιματολογικό, ανοσοφαινοτυπικό και κυτταρογενετικό \nεπίπεδο. Από την άλλη μεριά, τα αποτελέσματα από τη σύγκριση των \nχαρακτηριστικών της MBL με τα αντίστοιχα χαρακτηριστικά ασθενών χρόνιων \nλεμφουπερπλασιών, υποδεικνύουν πως υπάρχει μια κοινή βιολογία σε όλες τις MBL. \nΟι τακτικοί επανέλεγχοι αποκάλυψαν δύο διακριτά πρότυπα Β-κινητικής και την \nύπαρξη κλωνικής εξέλιξης στο προκλινικό στάδιο. Τα περιστατικά που εμφάνισαν \nκλωνική εξέλιξη παρουσίασαν μεγαλύτερο κίνδυνο μετάπτωσης σε «επίσημη νόσο». Ο \nαυξημένος αριθμός των Β λεμφοκυττάρων κατά τη διάγνωση αναδείχτηκε επίσης ως \nαρνητικός προγνωστικός παράγοντας για την μετάπτωση σε νόσο.
