Η έκφραση των ογκοπρωτε?νών p53 και bcl-2 μελετήθηκε σε 121 δείγματα μονιμοποιημένων σε παραφίνη βιοψιών ασθενών με μελαγχρωματικούς όγκους που περιελάμβαναν όλο το εύρος εξελίξεως, από σπίλους έως μεταστατικό μελάνωμα σε λεμφαδένες εκλεκτικής λεμφαδενεκτομής. Χρησιμοποιήθηκε η ανοσο?στοχημική μέθοδος και τα αντισώματα DO-7 και κλώνος 124, αντιστοίχως. Η έκφραση του p53 στους καλοήθεις όγκους και το μελάνωμα in-situ ήταν ασθενής, το δε ποσοστό των p53-θετικών κυττάρων κυμάνθηκε από 0%- 20%. Αντιθέτως, η έκφραση του p53 στο μελάνωμα ήταν έντονη, το δε ποσοστό των p53-θετικών κυττάρων κυμάνθηκε από 0%-98%. Το 30.2% των πρωτοπαθών και το 52% των μεταστατικών όγκων παρουσίασαν ποσοστό p53-θετικών κυττάρων άνω του 15%. Η έκφραση του p53 στον πρωτοπαθή όγκο δεν συσχετίσθηκε με καμμία προγνωστική παράμετρο, ενώ στον μεταστατικό όγκο η έκφραση του p53 συσχετίσθηκε ισχυρά με την αντίστοιχη έκφραση του p53 στον σύστοιχο πρωτοπαθή όγκο (p: 0.0001). Οι καλοήθεις μελαγχρωματικοί όγκοι και τα μελανώματα in-situ παρουσίασαν διάχυτη ανοσοθετικότητα γιά την πρωτεΐνη bcl-2 σε ποσοστό 100%. Αντιθέτως, παρατηρήθηκε απώλεια της έκφρασης του bcl-2 στο 45.3% των πρωτοπαθών και στο 60% των μεταστατικών μελανωμάτων. Η απώλεια της έκφρασης του bcl-2 ήταν μεγαλύτερη στα οζώδη, σε σχέση με τα επιφανειακώς εξαπλούμενα μελανώματα σε πολυπαραγοντική ανάλυση (p: 0.0345). Οι ασθενείς με ποσοστό p53-θετικών κυττάρων άνω του 15% είχαν δυσμενέστερη συνολική επιβίωση (p: 0.0057) και ελεύθερο-νόσου διάστημα (p:0.0083) από τους υπόλοιπους ασθενείς. Η πολυπαραγοντική ανάλυση κατά Cox ανέδειξε το πάχος (p: 0.0078) και την έκφραση του p53 στον πρωτοπαθή όγκο (p: 0.0366) ως ανεξάρτητους προγνωστικούς παράγοντες της συνολικής επιβιώσεως των ασθενών. Η έκφραση του bcl- 2, σε συνδυασμό με την έκφραση του p53 φαίνεται οτι διαφοροποίησε την κυτταροκινητική των μελανωμάτων, την δυσμενέστερη δε πρόγνωση είχαν οι ασθενείς με θετική έκφραση του p53 και απώλεια της έκφρασης του bcl-2. Τα ανωτέρω ευρήματα συνηγορούν υπέρ ενός πρώ?μου ρόλου του p53 στην εξέλιξη και μεταλλαξιογένεση του μελανώματος.- (EL)
The role of the oncoproteins p53 and bcl-2 was studied in 121 samples of paraffin-embedded melanocytic tumors, including a range of nevi to metastatic melanoma in lymph nodes, retrieved from elective lymphadenectomies. We used an immunohistochemical approach and the monoclonal antibodies DO-7 and clone 124, respectively. The expression of p53 on benign tumors and tissues of melanoma in-situ was weak and unstable. The percentage of p53-expressing cells ranged from 0% to 20% in these samples. On the contrary, the expression of p53 in melanoma tissues was intense, with the percentage of p53-expressing cells ranging from 0% to 98%. The percentage of p53-positive cells was higher than 15% in 30.2% of the primary, and 52% of the metastatic melanomas, respectively. The expression of p53 in the primary melanoma tumor did not correlate with any clinical or pathological variable. The expression of p53 in the metastatic tumor, strongly correlated with the respective expression of this molecule in the corresponding primary tumor (p: 0.0001). The benign melanocytic tumors and the melanomas in-situ diffussely and strongly expressed the bcl-2 protein. On the contrary, a loss of bcl-2- protein expression was obseved in 45.3% of the primary, and in 60% of the metastatic melanoma tumors. This loss of expression was more intense in the nodular, compared with the superficial-spreading primary melanomas, in multivariate analysis (p:0.0345). Disease-free and overall survival were worse in the patients whose primary melanoma tumor expressed the p53 protein in more than 15% of the tumor cells (positive expression), compared with the rest, in univariate analysis (p: 0.0083 and p: 0.0057, respectively). The thickness of the primary tumor and the expression of p53 in the primary tumor were independent prognostic variables for disease-free and overall survival in the Cox proportional hazards method (p: 0.0078 and p: 0.0366 respectively). The expression of bcl-2 protein in the primary tumor, in conjunction with the expression of p53 appeared to influence melanoma cell kinetics. The patients with positive p53-expression and loss of bcl-2 expression in their primary tumor had the worse prognosis. These findings suggest an early role of the p53 protein in the evolution and mutagenesis of melanoma.- (EN)
