Cardiovascular autonomic neuropathy (CAN) is a common and early complication of Diabetes Mellitus. It is a major contributor to the increased morbidity and mortality rates in the diabetic population. Both sudden death and silent myocardial ischaemia have been attributed to CAN. It can also cause many other problems to diabetic patients. Not long ago, CAN was believed to be a late complication of diabetes. Newer techniques have been developed for the detection of CAN, recently and it has become clear that autonomic function may become abnormal early in the course of diabetes. The clinical significance of CAN has been well recognized and established, recently. The role of good glycaemic control has been determined to development and progression of CAN in the latest years. Many pharmaceutical agents, which are active on major pathophysiological pathways of diabetic neuropathy, have been tested in clinical studies with promising results.
