The traditional clinical education and the evaluation of clinical skills present several restrictions and weaknesses. The new trends in medical education propose the need for support of clinical education and revision of the examination system. The growth of Clinical Skills Laboratories (CSL) for the training in clinical skills and their assessment with the Objective Structured Clinical Examination (OSCE), constitute two of the basic innovations towards the direction of clinical education improvement. In the CSL, the students are trained to basic clinical skills in conditions of simulation, in a protected environment, with a systematic and safe way, before the contact with real patients occurs. The OSCE normally includes a series of stations, with duration of 5-10 minutes each. The main skills examined at the stations of OSCE are history taking, communication skills, clinical examination, practical procedures and the interpretation of results with the contribution of simulated patients. A detailed mark scheme and standard set of questions are used. The Medical School of the Aristotle University of Thessaloniki (Medical School, AUTH), adopting the international tendencies in the medical education, has strengthened the training in the clinical skills with the operation of the CSL, which constituted the base for the next step: the study of OSCE itself, leading to the improvement of assessment methods of clinical skills. Σκοπός: Σκοπός της παρούσας έρευνας είναι η στάθμιση και η αξιολόγηση της Αντικειμενικά Δομημένης Κλινικής Εξέτασης (εξέταση OSCE) για πρώτη φορά σε Ιατρική Σχολή της Ελλάδας. Πληθυσμός και μέθοδος: Η μελέτη πραγματοποιήθηκε σε δύο φάσεις για την αξιολόγηση 74 τελειοφοίτων φοιτητών ιατρικής, με εκπαίδευση στην κλινική άσκηση της Παθολογίας, καθώς και 130 φοιτητών μικρότερων ετών. Όλοι οι φοιτητές εκπαιδεύτηκαν στο Εργαστήριο Κλινικών Δεξιοτήτων-ΕΚΔ. Μελετήθηκαν τα χαρακτηριστικά της μεθόδου καθώς και η διαφορά στην επίδοση, στην εξέταση OSCE πριν και μετά την εκπαίδευση σε κλινικές δεξιότητες. Επίσης, ζητήθηκε από τους φοιτητές να αξιολογήσουν την εξέταση OSCE και να την συγκρίνουν με τις παραδοσιακές γραπτές και προφορικές εξετάσεις. Αποτελέσματα: Η σύγκριση της βαθμολογίας της εξέτασης OSCE έδειξε στατιστικά σημαντική διαφορά στη βαθμολογία των φοιτητών πριν (Μ=36,83% SD=12,25), σε σχέση με τη βαθμολογία των φοιτητών μετά την εκπαίδευση στο ΕΚΔ (Μ=81,65% SD=6,09). Η διαφορά της βαθμολογίας στο σύνολο των σταθμών είναι 44,82% (95%CI 40,65-49,01). Ο σχεδιασμός της εξέτασης OSCE αξιολογήθηκε θετικά. Σε σύγκριση με τις παραδοσιακές γραπτές και προφορικές εξετάσεις η εξέταση OSCE κρίθηκε ως η πιο αδιάβλητη (83/196=42,3%, 95%CI 35,6-49,3), η πιο αντικειμενική για την αξιολόγηση κλινικών δεξιοτήτων (164/199=82,4% 95%CI 76,5-87 1) που πρέπει να χρησιμοποιείται περισσότερο στα κλινικά έτη σπουδών (103/192=53,6% 95%CI 46,6-60,6). Συμπεράσματα: Η εξέταση OSCE είναι μια αξιόπιστη, έγκυρη και εφαρμόσιμη μέθοδος για την εξέταση φοιτητών της Ιατρικής Σχολής του ΑΠΘ στις κλινικές δεξιότητες συνδυάζοντας υψηλό βαθμό αποδοχής από τους φοιτητές.
