Σκοπός: να πιστοποιήσουμε τις χαμηλές τιμές της CCSP 16 σε παιδιά με άσθμα και να διερευνήσουμε αν οι τιμές της στο αίμα μπορούν να διακρίνουν παιδιά με εκπνευστικό συριγμό από εκείνα που θα αναπτύξουν βρογχικό άσθμα. Υλικό-Μέθοδος: Υλικό της μελέτης αποτελούν 185 παιδιά ηλικίας 0-14 ετών, χωρισμένα σε 3 ομάδες. Ομάδα Α: Βρέφη και παιδιά (n=46) με συμπτώματα εκπνευστικού συριγμού κατά την εξέταση και παιδιά με ιστορικό συμπτωμάτων εκπνευστικού συριγμού-βρογχόσπασμο-βήχα για περισσότερο από μία εβδομάδα ή και χρήσης εισπνεόμενων βρογχοδιασταλτικών, κορτιζόνης ή κορτιζόνης από το στόμα, τα οποία πληρούν τα κλινικά κριτήρια για βρογχικό άσθμα. Ομάδα Β: Βρέφη και παιδιά (n=45) με συμπτώματα κατά την εξέταση που δεν πληρούν τα κλινικά κριτήρια για βρογχικό άσθμα. Ομάδα Γ: Βρέφη και παιδιά (n=94) τα οποία ουδέποτε είχαν συμπτώματα εκπνευστικού συριγμού- βρογχόσπασμο- βήχα για περισσότερο από μία εβδομάδα ή και χρήση εισπνεόμενων βρογχοδιασταλτικών, κορτιζόνης ή κορτιζόνης από το στόμα. Αυτή η ομάδα αποτελεί την ομάδα μαρτύρων (control) . Ο προσδιορισμός της CCSP 16 έγινε με ανοσοενζυμική μέθοδο ELISA. Για τη σύγκριση των ομάδων ως προς τα ποσοτικά χαρακτηριστικά χρησιμοποιήθηκε η δοκιμασία ANOVA, ενώ ως προς τα ποιοτικά χαρακτηριστικά η δοκιμασία χ2. Αποτελέσματα: Από την στατιστική ανάλυση φαίνεται ότι υπάρχει στατιστικά σημαντική διαφορά ανάμεσα στην ομάδα παιδιών μεγάλου κινδύνου για βρογχικό άσθμα συγκριτικά με την ομάδα παιδιών – υγιών μαρτύρων (p< 0.001). Η ομάδα μεγάλου κινδύνου για άσθμα παρουσιάζει χαμηλότερο μέσο όρο επιπέδων ορού CCSP 16 (16.4±12.9ng/ml, με διακύμανση 12.6-20.3 ng/ml), σε σύγκριση με τους υγιείς μάρτυρες (31.9±19.7 ng/ml, με διακύμανση 26.0-37.8 ng/ml). Φαίνεται ότι αυτά τα πρώτα αποτελέσματα πιστοποιούν τις χαμηλές τιμές της πρωτεΐνης σε παιδιά με βρογχικό άσθμα. Επιπλέον, οι έλεγχοι συσχετίσεων που πραγματοποιήθηκαν έδειξαν την αναμενόμενη στατιστικά σημαντική και ισχυρή συσχέτιση του άσθματος με όλα τα συμπτώματα που έχουν καταγραφεί, ενώ δεν αποδείχθηκαν σημαντικές οι συσχετίσεις που αφορούν στην κληρονομικότητα ή στο κάπνισμα. Ο έλεγχος του μέσου όρου της πρωτεΐνης στις τρεις ομάδες ασθενών και οι πολλαπλές συγκρίσεις μεταξύ τους έδειξαν ότι η CCSP 16 έχει χαμηλότερες, κατά μέσο όρο, τιμές στους ασθενείς από ότι στις άλλες δύο ομάδες, οι οποίες δεν διαφέρουν σημαντικά μεταξύ τους. Στο αποτέλεσμα αυτό οδήγησαν τόσο οι παραμετρικοί όσο και ο μη παραμετρικός έλεγχος. Η λογιστική παλινδρόμηση και η απεικόνισή της με τη χρήση των εμπειρικών καμπύλων ROC έδειξαν ότι η διαχωριστική ικανότητα μεταξύ της ασθένειας και της μη ασθένειας, που παρέχεται από την πρωτεΐνη CCSP 16, διαφέρει σημαντικά από την τυχαία και είναι ανεπηρέαστη από την ηλικία του πληθυσμού στην οποία εφαρμόστηκε. Συμπεράσματα: Τα παιδιά με άσθμα παρουσιάζουν στατιστικά σημαντικά χαμηλότερες τιμές της πρωτεΐνης CCSP 16 από τους μη πάσχοντες. Συνεπώς, οι τιμές της πρωτεΐνης στο αίμα μπορούν να διακρίνουν ανάμεσα στα παιδιά με εκπνευστικό συριγμό εκείνα που θα αναπτύξουν βρογχικό άσθμα.
Purpose: To certify the low values of CCSP 16 in children with asthma and to investigate if protein values in blood may determine children with wheezing that will progress to bronchial asthma. Subject-Methods: It was conducted a study of 185 children, aged between 0-14 years and divided into 3 groups. Group A: infants and children (n=46) with wheezing symptoms during examination and children with a history of wheezing symptoms, bronchospasm and cough for more than a week and/ or of inhaled bronchodilators, steroids or oral steroids, that perform the criteria for bronchial asthma. Group B: infants and children (n=45) with symptoms during examination that do not fulfill the clinical criteria for bronchial asthma. Group C: infants and children (n=94) who have never presented symptoms of wheezing, bronchospasm or cough for more than a week and/ or have never used inhaled bronchodilators, steroids or oral steroids. This is the control group. CCSP 16 levels were determined by ELISA. The ANOVA test was used for quantitative comparison of the group’s characteristics, whereas, the qualitative characteristics were tested by χ2. Results: From the initial analysis, it seems that there is significant statistical difference between the high risk bronchial asthma group compared to the control group of children (p<0.001). The high risk for bronchial asthma group has lower median values of CCSP 16 serum levels (16.4± 12.9 ng/ml, range 12.6-20.3 ng/ml) compared to the control group (31.9±19.7 ng/ml, range 26.0-37.8 ng/ml). The initial results confirm a correlation between the low values of the protein and bronchial asthma in children. Moreover, the tests showed the expected statistically significant and strong correlation of asthma to all the described symptoms. However, no significant correlation between hereditary factors and/or smoking was identified. Testing of the median values of protein in the 3 groups and multiple correlation checks between than showed that CCSP 16 has lower average values in patients compared to the other 2 groups who do not significantly differ from each other. This conclusion was reached by both parametric and non parametric tests. Logistic regression and its group with the use of ROC experimental charts showed that the ability to segregate between disease and non-disease provided by CCSP 16 protein, differs significantly from randomness and is not affected by the population age it applied to. Conclusion: Children with asthma show statistically, significantly lower CCSP 16 protein values compared to healthy ones. Consequently, protein values in blood may determine children with wheezing that will progress to bronchial asthma.
