The purpose of this study was to develop a pharmaceutical drug delivery system which will be appropriate for intravenous administration for the targeting release of the anticancer active ingredient paclitaxel during chemotherapies in combination with the application of mild hyperthermia to the tumor site. The proposed drug delivery system consisted of polymer-paclitaxel nanoparticles for which it was desired to show low paclitaxel release rates at 37oC and higher release rates at 42oC in order to achieve low toxicity levels to physiological human cells but higher effectiveness against cancer cells which will be topically heated. Therefore biocompatible polymeric carriers where prepared which were aliphatic polyesters like poly(propylene adipate) (PPAd) of different molecular weights and their copolymers with methoxy polyethylene glycol (mPEG) of different molecular weights. The innovation of these materials was the fact that they are biocompatible, showing lower or equal cytotoxicity compared to poly(lactic acid) which is already commonly used for similar applications and further the fact that they have melting point near to the temperature of human body. Further, aliphatic polyesters like poly(propylene pimelate) and copolymer of poly(ethylene succinate) and poly(propylene adipate) were also prepared.For the encapsulation of paclitaxel in the nanoparticles the oil in water emulsification and solvent evaporation technique was used which showed high reproducibility and satisfactory drug loading and entrapment efficiency. The prepared nanoparticles showed a spherical shape with a mean diameter which varied from 80 up to 300nm, hence they were appropriate for intravenous administration. Especially nanoparticles prepared by mPEG-PPAd copolymers were self-assembled in the form of core-shell nanospheres in which the hydrophilic mPEG chains form an external hydrophilic shell and the hydrophobic PPAd chains form an internal hydrophobic core. Paclitaxel which is hydrophobic lies mostly in the core of the nanoparticles but also some quantities are adsorbed on the nanoparticle surface. The dissolution studies of paclitaxel from the nanoparticles were performed at temperatures of 37oC and 42oC in pH7.4 (blood simulation) and pH6.0 (endosomal pathway simulation). For both pH conditions the release behavior of paclitaxel was similar since an initial burst release of paclitaxel which was adsorbed on the nanoparticle surface was obtained followed by sustained release due to the diffusion of paclitaxel from the core of the nanoparticles. However it was observed that at pH6.0 the release rate of paclitaxel is higher compared to pH7.4, especially for the nanoparticles prepared by aliphatic polyesters, due to the easier hydrolysis of the ester bonds which can be obtained at pH6.0. Furthermore, it was observed that all the studied materials were thermosensitive since they showed generally higher release rates of paclitaxel at 42oC compared to 37oC. The observed thermosensitivity was higher for the nanoparticles which had comparatively higher degree of crystallinity.Finally, according to the in-vitro cytotoxicity studies it was shown that all the prepared polymer-paclitaxel nanoparticles exhibited high toxicity against cancer cells. The effect of the nanoparticles against the HeLa cancer cells was higher compared to the HepG2 cancer cells. Further the necrosis/apoptosis of the cancer cells was observed faster at 42oC compared to 37oC which is another indication that the studied materials can act as thermosensitive drug carriers.
Ο στόχος της διδακτορικής διατριβής ήταν η ανάπτυξη φαρμακευτικού σκευάσματος κατάλληλου για ενδοφλέβια χορήγηση που να επιτυγχάνει ‘στοχευμένη’ αποδέσμευση της αντικαρκινικής δραστικής paclitaxel κατά τη χημειοθεραπεία ασθενών με ταυτόχρονη εφαρμογή τοπικής υπερθερμίας στον καρκινικό όγκο. Το προτεινόμενο φαρμακευτικό σκεύασμα αποτελείται από νανοσωματίδια πολυμερούς-paclitaxel τα οποία ήταν επιθυμητό να εμφανίζουν χαμηλή αποδέσμευση της δραστικής στους 37oC και υψηλότερη αποδέσμευση στους 42oC επιτυγχάνοντας χαμηλή τοξικότητα σε φυσιολογικούς ιστούς αλλά υψηλή αντικαρκινική δράση στους καρκινικούς όγκους που θα θερμαίνονται τοπικά.Παρασκευάστηκαν κατάλληλοι βιοσυμβατοί πολυμερικοί φορείς που ήταν αλειφατικοί πολυεστέρες όπως ο πολυ(αδιπικός προπυλενεστέρας) (PPAd) διαφόρων μοριακών βαρών και συμπολυμερή αυτού με μεθόξυ-πολυαιθυλενογλυκόλη (mPEG) διαφόρων μοριακών βαρών. Η πρωτοτυπία αυτών ήταν ότι είναι βιοσυμβατά υλικά, αφού εμφανίζουν κυτταροτοξικότητα χαμηλότερη ή παραπλήσια από αυτή του πολυ(γαλακτικού οξέος) το οποίο χρησιμοποιείται εκτεταμένα σε ανάλογες εφαρμογές και εμφανίζουν σημείο τήξεως κοντά στην θερμοκρασία του ανθρώπινου σώματος. Επίσης παρασκευάστηκαν παρόμοιοι αλειφατικοί πολυεστέρες όπως ο πολυ(πιμελικός προπυλενεστέρας) και το συμπολυμερές πολυ(ηλεκτρικού αιθυλενεστέρα)- πολυ(αδιπικού προπυλενεστέρα).Για τη νανοενθυλάκωση της paclitaxel επιλέχθηκε η τεχνική γαλακτωματοποίησης ελαίου/ύδατος και εξάτμισης διαλύτη η οποία παρουσίασε μεγάλη επαναληψιμότητα και ικανοποιητικά ποσοστά απόδοσης και ενθυλάκωσης της δραστικής. Τα νανοσωματίδια που παράχθηκαν ήταν σφαιρικού σχήματος με μέγεθος κατάλληλο για ενδοφλέβια χορήγηση (80-300nm). Ειδικότερα, τα νανοσωματίδια που παρασκευάστηκαν με συμπολυμερή mPEG-PPAd είχαν τη μορφή νανοσφαιριδίων με διάταξη μήτρας-κελύφους (core-shell) στα οποία οι υδρόφιλες συστάδες mPEG διατάσσονται στο εξωτερικό δημιουργώντας μια εξωτερική στιβάδα (κέλυφος), ενώ οι υδρόφοβες συστάδες PPAd διατάσσονται στο εσωτερικό τους δημιουργώντας μια εσωτερική μήτρα. Η paclitaxel που είναι υδρόφοβη βρίσκεται κυρίως στο εσωτερικό των νανοσωματιδίων ενώ κάποια ποσότητά της βρίσκεται προσροφημένη και στην επιφάνεια των νανοσωματιδίωνΗ μελέτη της αποδέσμευσης της paclitaxel από τα νανοσωματίδια πραγματοποιήθηκε σε θερμοκρασίες 37οC και 42οC και σε pH7.4 (συνθήκες αίματος) αλλά και pH6.0 (συνθήκες ενδοσωματικού δικτύου). Και στα δύο pH η συμπεριφορά αποδέσμευσης της paclitaxel από τα νανοσωματίδια ήταν παρόμοια καθώς αρχικά παρατηρείται έντονη αποδέσμευση της paclitaxel που βρίσκεται προσροφημένη στην επιφάνεια και στην εξωτερική πλευρά των νανοσωματιδίων ακολουθούμενη από σταδιακή αποδέσμευση που οφείλεται στη διάχυση της δραστικής που βρίσκεται στο εσωτερικό των νανοσωματιδίων. Ωστόσο, σε pH6.0 παρατηρήθηκε αυξημένος ρυθμός αποδέσμευσης του φαρμάκου συγκριτικά με pH7.4, ιδιαίτερα για την περίπτωση των αλειφατικών πολυεστέρων καθώς σε pH6.0 ευνοείται η υδρόλυση των εστερικών δεσμών. Επίσης γενικά όλα τα υλικά που μελετήθηκαν έδρασαν ως θερμοευαίσθητα εμφανίζοντας μεγαλύτερη αποδέσμευση στους 42oC συγκριτικά με τους 37oC. Ειδικότερα, μεγαλύτερη θερμοευαισθησία εμφάνισαν τα νανοσωματίδια που είχαν το μεγαλύτερο βαθμό κρυσταλλικότητας. Από τις in-vitro μελέτες κυτταροτοξικότητας βρέθηκε ότι όλα τα νανοσωματίδια πολυμερούς-paclitaxel εμφάνισαν υψηλή τοξικότητα απέναντι σε καρκινικά κύτταρα. Η επίδραση των νανοσωματιδίων πολυμερούς-paclitaxel ήταν πιο έντονη απέναντι στη σειρά καρκινικών κυττάρων HeLa συγκριτικά με τα καρκινικά κύτταρα HepG2 ενώ μπόρεσε να παρατηρηθεί και σε αυτές τις μελέτες σχετικά πιο γρήγορη επίδραση στους 42oC συγκριτικά με τους 37oC κάτι που αποτελεί επιπλέον ένδειξη της θερμοευαισθησίας νανοσωματιδίων.
