The aim of this study was to investigate whether the tissue polypeptide-specific antigen (TPS) levels in cystic fluid, peritoneal fluid, and blood could be used as a marker for the management of ovarian mass. Forty-one patients participated in the study with preoperative diagnosis of an ovarian mass. The patients were divided into two groups according to the results of the histopathologic examination. Group 1 included patients with a benign ovarian mass, and Group 2 included patients with ovarian cancer. Blood samples were taken just before surgery, and cystic fluid and peritoneal fluid were taken at the time of the operation. Comparison of the results between the two groups was performed with the Wilcoxon signed-rank test. There was a statistically significant difference between the TPS serum values in Group 1 compared with Group 2 (p = 0.004). Also, the mean levels of TPS in the serum of patients in Group 2 were significantly higher compared with the cutoff value provided by the manufacturer (80 U/L) and compared with the mean levels of TPS of patients in Group 1. Also, there was a statistically significant difference between the TPS values of the cystic fluid between the two groups (p < 0.05). This study suggests that serum TPS values can be used as a sensitive marker in differentiating malignant from benign ovarian cysts. (EN)
