This retrospective study deals with the primary surgical treatment of 15 isolated avulsion fractures of the anterior tibial spine (eminence) and the results after 1-4 years follow-up. The incidence of the injury seems to grow every year due to traffic accidents and athletic injuries. The mechanism of injury was either violent posterior displacement of the femoral condyles with the knee flexed, or anterior displacement of the tibia with the knee hyperextended, or violent abduction of the knee with the tibia in external rotation. The X-rays (plain and tomograms) and a positive anterior drawer sign under anaesthesia (stress films) gave the indication for primary O.R.I.F. The fracture was reduced to its anatomical position and held in place by the pull-out technique, or with a screw. After 1-4 years follow-up our results were 12 excellent, 2 good and 1 fair. © 1993 Springer-Verlag. (EN)
