Εισαγωγή: Η εκτίμηση της ποιότητας ζωής (ΠΖ) ασθενών με μόνιμο καρδιακό βηματοδότη αποτελεί πολύτιμο εργαλείο για την αναγνώριση της αποτελεσματικότητας της θεραπείας της καρδιακής βηματοδότησης στην καθημερινή λειτουργία των ασθενών.
Σκοπός: Αξιολόγηση, μέσω συστηματικής ανασκόπησης της διεθνούς βιβλιογραφίας, της επίδρασης της εμφύτευσης μόνιμου βηματοδότη στην ποιότητα ζωή των ασθενών.
Υλικό και μέθοδος: Η αναζήτηση των άρθρων πραγματοποιήθηκε στις βιβλιογραφικές βάσεις δεδομένων PubMed και Cochrane, με τη χρήση των ακόλουθων λέξεων-κλειδιών: «quality of life», «permanent pacemaker», «pacemaker implantation» και «cardiac devise». Οι λέξεις-κλειδιά χρησιμοποιήθηκαν μόνες και σε συνδυασμό. Τα κριτήρια επιλογής για την αναζήτηση των άρθρων ήταν τα ακόλουθα: α) μελέτες δημοσιευμένες εντός της τελευταίας δεκαετίας, β) τυχαιοποιημένες ελεγχόμενες δοκιμές δημοσιευμένες στην αγγλική γλώσσα και γ) μελέτες σε ανθρώπους ενήλικες (>18 ετών), ανεξαρτήτως φύλου.
Αποτελέσματα: Βρέθηκαν συνολικά 119 μελέτες, από τις οποίες οι 9 πληρούσαν τα κριτήρια ένταξης στην παρούσα μελέτη. Στις μελέτες, αξιολογήθηκε η ΠΖ ασθενών με μόνιμο βηματοδότη, μέσω ερωτηματολογίων και κλιμάκων μέτρησης. Σε όλες τις μελέτες παρατηρήθηκε η ευεργετική επίδραση της εμφύτευσης μόνιμου βηματοδότη στην ΠΖ των ασθενών. Πιο συγκεκριμένα, η οικογενειακή κατάσταση και η ηλικία επιδρά στην ΠΖ των ασθενών, καθώς οι έγγαμοι ασθενείς παρουσίασαν υψηλότερη ΠΖ, ενώ οι ηλικιωμένοι ασθενείς έφειξαν πτωχότερη ΠΖ. Επιπλέον, υπήρξε θετική συσχέτιση μεταξύ της γνώσης και της βοήθειας στις καθημερινές δραστηριότητες, από τους επαγγελματίες υγείας, καθιστώντας τον ρόλο των κλινικών νοσηλευτών πολύ σημαντικό. Τέλος, το αίσθημα της μη εξάρτησης από την συσκευή του μόνιμου βηματοδότη παρατηρήθηκε ότι συμβάλει τόσο στην λειτουργικότητα των ασθενών, όσο και στην βελτίωση της ΠΖ των ασθενών.
Συμπεράσματα: Οι μελέτες που συμπεριλήφθηκαν στην παρούσα συστηματική ανασκόπηση της διεθνούςβιβλιογραφίας, έδειξαν βελτίωση στην ΠΖ και στους παράγοντες που σχετίζονται με αυτή, των ασθενών με μόνιμο βηματοδότη, παρέχοντας δεδομένα για παρεμβάσεις με στόχο τη διευκόλυνση των θετικών προσαρμογών μετά την εμφύτευση της βηματοδότη.

Introduction: The QoL is a multidimensional phenomenon which reflects the impact of the condition, the clinical picture and the treatment of the condition on the daily life of the patients. Assessing the quality of life of patients with a permanent cardiac pacemaker can help doctor and nurses better identify the cardiac pacing therapy impact on the patients' daily functioning. The implantation of a permanent pacemaker positively influences patients' QoL, by improving their physical and mental health and social functioning.
Aim: The aim of this study was to assess the influence of permanent cardiac pacemaker implantation on the patients’ QoL, through systematic review of the international literature.
Material and Methods: The articles were searched in the PubMed and Cochrane bibliographic database using the following keywords: «quality of life», «permanent pacemaker», «pacemaker implantation» and «cardiac devise». The keywords were used alone and in combination. The selection criteria for the article search were the following: a) published studies in the last 10 years, b) published studies in English and c) patients aged above 18 years, of both genders.
Results: A total of 119 studies were found, 9 of which met the inclusion criteria for the present study.Seven were cross-sectional studies, one studyusedmethodology of mixed consisting of quantitative and qualitative data, and one was quasi-experimental study.In these studies, patients’ quality of life wasassessed using questionnaires and measurement scales. Questionnaires and measuring scales were the tools to gather data related to physical and mental health, as well as social functioning and the knowledge of the patients on the permanent pacemaker and its therapeutic intervention.All studies showed the beneficial effect of permanent pacemaker implantation on patients' QoL. More specifically, marital status and age were associated with patients' QoL. Married patients reported better QoL than unmarried
and a low QoL was reported be elderly patients. In addition, there was a positive correlation between knowledge and assistance in activities of daily living, from health professionals, making the role of clinical nurses very important. Finally, the feeling of not being dependent on the permanent pacemaker device was observed to contribute both to the patients' functionality and to the improvement of the patients' QoL.
Conclusions: This systematic review showed an improvement in the quality of life and the factors associated to it, in patients with a permanent pacemaker, providing key opportunities to nurses for interventions aimed at facilitating positive adaptations following permanent pacemaker implantation.
