Ο τράχηλος είναι µια περιοχή του ανθρωπινού σώµατος µε ιδιαίτερο ενδιαφέρον και δυσκολία στην προσπέλαση της καθότι «βρίθει» ευαίσθητων ανατοµικών δοµών. Μια από αυτές είναι το άνω λαρυγγικό νεύρο και ιδιαίτερα ο έξω κλάδος του, το οποίο παρότι είναι γνωστό από παλιά, ακόµα προκαλεί το αµείωτο ενδιαφέρον ενός ευρέως φάσµατος επιστηµόνων. Κάθε τόσο νέες ανατοµικές µελέτες έρχονται στη δηµοσιότητα τόσο για να επιβεβαιώσουν, όσο και για να αναθεωρήσουν τις προηγούµενες, προσθέτοντας ένα «µικρό λιθαράκι» γνώσης στην ήδη υπάρχουσα βιβλιογραφία. Οι κλινικοί γιατροί, άλλα ακόµα και διάφοροι άλλοι επιστήµονες, όπως οι λογοθεραπευτές, έχουν αρχίσει δειλά δειλά να αναγνωρίζουν την αξία αυτής της ανατοµικής δοµής. Αυτό το ανατοµικό στοιχείο είχε παραγνωρισθεί στο παρελθόν, και είχε χαρακτηρισθεί από κάποιους σαν το “εγκαταλελειµµένο νεύρο” της χειρουργική κεφαλής και τραχήλου (Lennquist et al.1997, Delbridge 2001). Για τούτο σκεφτήκαµε να µελετήσουµε καλύτερα την ανατοµική του πορεία και την κλινική του σηµασίας . Στο Γενικό Μέρος αυτής της µελέτης, µετά την αναλυτική παρουσίαση της ανατοµίας και φυσιολογίας της περιοχής που νευρώνει το άνω λαρυγγικό νεύρο, γίνεται αναφορά στις επεµβάσεις κατά τις οποίες µπορεί να τρωθεί το νεύρο. Επιπλέον γίνεται ενδελεχής αναφορά στην διαγνωστική προσέγγιση των κακώσεων και των παραλύσεων του νεύρου, καθώς και στην κλινική σηµασία αυτών κατά την διάρκεια της φώνησης και της κατάποσης. Αναφέρονται οι παρατηρήσεις µας στον συγκεκριμένο τοµέα προσεγγίζοντας τις πιθανές θεραπείες αποκατάστασης. Η στενή σχέση της δοµής αυτής µε τον θυρεοειδή αδένα και τις σχετιζόµενες µε αυτόν επεµβάσεις περιγράφονται αναλυτικά, θεωρώντας πολύ σηµαντική την σχέση αυτή και η γνώση της αποτελεί βοήθεια κατά τις χειρουργικές αυτές επεµβάσεις. Στο Ειδικό Μέρος αυτής της µελέτης παρουσιάζονται αναλυτικά το υλικό, η µεθοδολογία που ακολουθήθηκε και τα αποτελέσµατα από την στατιστική επεξεργασία των µετρήσεών µας. Τα συµπεράσµατα µε τα οποία ολοκληρώνεται η µελέτη αυτή, µας οδηγούν στην καλύτερη αναγνώριση και διατήρηση των νεύρων κατά τη διάρκεια των χειρουργικών επεµβάσεων της περιοχή του τραχήλου, καθώς και στην έγκαιρη διάγνωση όποιας νοσολογίας σχετίζεται µε το νεύρο αυτό. Material: The study was conducted at the Laboratory of Anatomy, Medical School of Athens University. For this purpose we used 42 cadavers bilaterally (Hemi-larynx) of the 2 sexes. The anatomical dissections began in October 2005 and completed in October 2008. In 2005 the methodology was established. Method: Firstly under the guidance of Prof. Anagnostopoulou (Anatomist) the protocol, of the anatomic cadaver dissections, was established. The Cadavers were placed in the supine position and with the help of a wooden pad between the shoulders, the neck was hyperextended. The protocol included the following steps: An incision is made in the midline of the anterior cervical triangle. The skin and platysma were dissected and removed. Then the anterior edge of the sternocleidomastoid muscle and following infrahyoid muscles are dissected: 1) omohyoid, 2) sternothyroid, 3) sternohyoid, 4) thyrohyoid. The infrahyoid muscles are separated in the middle (a white line). The sternohyoid muscle is pulled firmly laterally and the sternothyroid muscle is incised near the thyroid cartilage. Thus the superior pole of the thyroid gland is revealed and an overview of the site is unobstructed. Afterwards the upper pole of the thyroid gland is dissected. We detach the upper pole of the thyroid gland from the cricothyroid muscle and the lower sphincter of the pharynx. 2.5 x magnifying surgical loops were used during the procedure. At that point we began the dissection of the superior thyroid blood vessels and the search for the external branch of the superior laryngeal nerve. The blood vessels were dissected separately. The external branch of the superior laryngeal nerve was searched near its possible entry in the cricothyroid muscle, in the inferior constrictor of the pharynx and all the way up to the carotid sheath. After we identified the point where the external branch of the superior laryngeal nerve was crossing the superior thyroid artery, if there was any. Using a calliper, the distance between the superior pole of the thyroid gland with that point, was accurately measured. Photos of the external branch of the superior laryngeal nerve and the nearby anatomical structures were taken. The measurements were used to classify the nerves according to Cernea’s and Friedman’s classifications. Cernea has classified the nerve into three types. Type 1: The external branch of the superior laryngeal nerve crosses the superior thyroid artery at a distance greater than 1 centimetre from the superior pole of the thyroid gland, Type 2a: The external branch of the superior laryngeal nerve crosses the superior thyroid artery at a distance less than 1 centimetre from the superior pole of the thyroid gland. Type 2b: The external branch of the superior laryngeal nerve crosses the superior thyroid artery lower than the level defined by the upper pole of the thyroid gland. Finally there is one category where the nerve may be not identified during the dissection. (No). Friedman’s classification describes three variations. In type I variation, the external branch of the superior laryngeal nerve runs its whole course superficial to the inferior constrictor of the pharynx it terminates in the cricothyroid muscle. In type II variation, the external branch of the superior laryngeal nerve penetrates the inferior constrictor of the pharynx at its lower portion. In type III variation, the external branch of the superior laryngeal nerve dives under the superior most fibres of the inferior constrictor and remains coved by this muscle throughout its course to the cricothyroid muscle. Results: This study was done in 21 cadavers, where he studied 42 hemilarynx. Using the above mentioned technique was easy to identify the nerve in 41 hemilarynx, i.e. 97.5% of cases, while in one case the identification of the nerve was not possible (2.5%). Based on our measurements, we found that the type 1 nerve (Cernea’s Classification) was present in 8 of the 42 cadavers, or 19% of the nerves were classified as type 1, type 2a variation was present in 23 of the 42 cadavers, i.e. 57% of the nerves were classified as type 2a, type 2b variation was found in 9 of 42 cases, namely 21.5% is classified in type 2b. In one case the nerve could not be detected (2.5%). This is due to the fact that this cadaver had undergone a lobectomy of the left lobe of the thyroid. The anatomy of the region was greatly disturbed, and the detection of the external branch of the superior laryngeal nerve was impossible, despite our strenuous efforts. Where the external branch of the superior laryngeal nerve passed cranial to the superior pole of the thyroid gland, i.e. 32 (76.5%) cases, the average distance from the superior pole of the gland was 7.43 mm. Based on Friedman’s classification we found in type I in 79% of the cases, the type II in 21% of the cadavers and type III in 0% of the cases. It is noteworthy that nerve had symmetrical course between the two sides of the neck (right-left) in 19% of the cases only. Although we have not searched thoroughly for those terminal branches, in two cases we found branches from the external branch of the superior laryngeal nerve terminating in the thyroid gland.
